Once you have scheduled an appointment, please download and complete the appropriate new patient paperwork and bring the packet with you to your appointment.
A few of our specialists only accept referred patients. Be sure to check with your insurance company to see if a referral is necessary. If you belong to an HMO such as Hill Physicians or Sutter Medical Group, you will need prior authorization from your Primary Care Physician listed on your insurance card. If you have insurance coverage that does not require a referral to be seen by a specialist, please give our office a call directly to determine if the physician you would like to see requires a referral in order to been seen as a new patient.
When contacting our office for an appointment, be sure to have your medical records available as well as your insurance card so that we may schedule you with the most appropriate provider for your medical condition. If you have to leave a message with one of our new patient coordinators, please allow 24-48 hours for a call back.
